WSUS updates getting stuck (SBS 2008)

I recently installed WSUS on an SBS 2008 server and have completed several synchronisations.  I have approved various updates for both the server and client PCs - and they have mostly been downloaded and installed.  But a few have got stuck, with no change of status for two or three weeks.  Could someone explain where I'm going wrong?  Here is an example. 

In June, I approved KB2813430 (Security Update for Windows Server 2008 x64 edition) but it is still not installed.  On the "All Updates" page in WSUS, if I filter just to show updates that are both "Approved" and "Needed", it appears in the list.  It has an Installed/Not Applicable Percentage of 86% and the Approval column says "Install (2/5)".

My questions are:

- 86% of what?  (i.e. I only have one server)

- does the word "Install" means "yet to be installed" or "is currently in the process of being installed"?

- what does "(2/5)" after the word "Install" mean?

I have checked under the "Additional Details" section.  This update supersedes a couple of other updates, but hasn't itself been superseded.  I therefore can't figure out why it won't install.
